WHAT IS INFERTILITY?

Infertility can be defined as difficulty in becoming pregnant (conceiving) despite having regular unprotected sex atleast for one year.

It can be primary or secondary. Primary infertility is when a pregnancy has never been achieved by a person, and secondary infertility is when at least one prior pregnancy has been achieved.

When should you visit your gynaecologist?

The most important aspect of treating infertility is timely diagnosis and intervention.

The couple must understand when to visit the doctor.

• Any couple unable to conceive after one year of unprotected intercourse.
• Age 35 years or more and have been trying to conceive for 6 months or longer.
• Are over the age of 40 years.
• Have irregular or absent periods.

What are the causes of infertility?

About one-third of the time (30%), it is a female problem. In another one-third of cases(30%), it is the man with the fertility problem. For the remaining one-third, both partners have fertility challenges or no cause is found(UNEXPLAINED INFERTILITY) which means everything is normal but still you are not able to get pregnant. Even in such cases,you should not wait for longer periods.Instead visit timely for intervention.

1. Ovulation problems- Not producing eggs (ovulating) is the cause of problems in about 1 in 4 couples.There can be many reasons for it like

• Age-Older women tend to be less fertile and have low ovarian reserve than younger women.

• Premature ovarian failure,
• excessive strenuous exercise,

2. Fallopian tube, cervix or uterine problems

These are the cause in about 2 or 3 in 10 couples with infertility. Problems include:

• Blocked fallopian tubes due in infections(PID),fluid in tubes(TB).

1. Blood tests -
Routine blood tests as well as hormonal parameters are the first tests offered for infertility by your gynaecologist to detect hormonal causes of infertility. They are mostly done on Day 2 -5 of your periods.

2. Husband Semen Analysis -

To deter quality and quantity of sperm count and hence semen analysis plays a very crucial role in infertility evaluation. Masturbation is the most common way used, to collect a sample where ejaculation is done into a sterile cup. A dedicated chamber is given to the male partner for semen collection(at pande hospital).

Things to keep in mind before giving the semen sample for evaluation

• To avoid sexual intercourse for 3-5 days
• No masturbation for 3-4 days
• No use of lubricants while producing the sample.
• In case you want to produce the sample at home, you have to deposit the sample within one hour after ejaculation of semen.

3. Ultrasound -

The primary use of imaging methods in infertility is to assist the doctor to find out the cause of couple’s infertility and to provide effective treatment modality. 4. Hysterosalpingogram/ HSG -

Our in house diagnostic centre offers Hystersalpingography to assess the patency of fallopian tubes.

An Xray is taken after injecting dye inside the uterus and films are taken to visualize passage of dye through the tubes.

5. Counselling -

Psychological counselling is necessary for patients undergoing infertility treatments to ensure that their anxiety and stress levels are not aggravated.

What are the treatment options for infertile couples?

Ovulation Induction

1. Ovulation Induction -

This is the most basic first step to initiate any form of treatment to an infertile couple.

In this we give medications to stimulate the ovaries to develop eggs which increases the chances of conception through times sexual intercourse or through artificial insemination(IUI).

What are the steps of ovulation induction?

• BASIC ASSESSMENT – The ovulation cycle is assessed through blood tests to measure hormonal levels at specific stages of your cycle. An Ultrasound (follicular study) is performed to see the development of follicles in the ovaries.

• STIMULATION – The ovaries are then stimulated with medicines to promote the growth of follicles. Ovulation is timed and patient is counselled for timed intercourse.

2. Intrauterine Insemination -

A type of artificial insemination — is a procedure for treating infertility. Sperm that have been washed and concentrated are placed directly in your uterus around the time your ovary releases one or more eggs to be fertilized. Depending on the reasons for infertility, IUI can be coordinated with your normal cycle or with fertility medications. 3. IVF/Test Tube Baby -

In women with blocked and damaged tubes usually the best option is test tube baby (IVF), which has very good results today. In Vitro Fertilization is a revolution in Assisted Reproductive Techniques (ART), where a woman’s eggs are retrieved from her ovaries and fertilized with a man’s sperm and fertilised externally. The embryo thus formed is infused back into the woman’s uterus to result in a fruitful pregnancy.

It is recommended when initial fertility treatments like INTRAUTERINE INSEMINATION or oral drugs do not bring any fruitful result.IVF is advised in cases of:
